Genomic Data Cross-Reference Files are required to link HRS data to the HRS genomic data stored data stored in the dbGaP and NIAGADS systems. Submit this form to apply for access to the cross-reference files after you receive dbGaP or NIAGADS approval. A completed and signed Genomic Data Access Use Agreement and a copy of your dbGaP or NIAGADS approval are required, in .pdf format.
